The most common failure is vagueness. Phrases like quality service, customer focused, and trusted partner appear on so many sites that they carry no information at all; a competitor could paste them onto their own page without anything looking wrong. A useful test is whether a direct competitor could claim the same sentence. If they could, it is a slogan, not a value proposition.
Strong versions are specific about the audience and the outcome. Naming a niche, a geography, a constraint you remove, or a situation you specialise in makes the statement testable, and testable claims are believable ones. Specificity also does useful filtering: telling the wrong prospects that you are not for them saves everyone time and improves the quality of the enquiries you do get.
A value proposition is a claim, and claims invite follow-up questions. Visitors want to know whether the promise holds for their particular situation, which is where most sites go quiet.
